What brought you here today?
Select the household’s most urgent need. Civic Trust will determine which connected services must change so that assistance can begin without separate applications.
Major household needs
These requests can require placements or changes across several participating services.
Routine changes and reviews
Smaller requests usually affect fewer services, though a record conflict or safety concern can expand review.
How should this request be handled?
Choose the approach that best reflects the household’s priorities.
Both options are available under current policy. The selected approach changes the services, timing, conditions, and household determinations in the final notice.

Civic Trust has removed the need for additional applications.
What your request changed
These determinations were made together. A change to one service may alter eligibility, priority, location, schedule, or household standing in another.
Participating services and providers have been notified.
Government services and approved providers receive only the records Civic Trust determines are necessary. Required participation supports continuous eligibility; choosing another provider does not preserve the allocations described in this plan.
Your correction and withdrawal rights
You may request correction of a name, household number, address, or recorded event. A correction request does not suspend this plan.
You may ask to withdraw the integrated application. Withdrawal is reviewed as a whole: individual benefits and connected determinations cannot be separated while review is pending, and previous eligibility is not automatically restored.
The primary benefit is temporarily suspended.
Your request to separate the integrated plan has been recorded for this demonstration. Because continuous eligibility depended on coordinated participation, the requested primary benefit is suspended during review.
Connected placement, reporting, inspection, and record-transfer determinations remain effective until withdrawal is approved. Services in place before this application are not automatically restored.
You may keep this notice for your records. Participating providers will contact the household only when an action cannot be completed automatically.
